Healthy Homes Programs
We believe that a quality, dignified, affordable place to call home is one of the least costly and most effective ways to deliver health care supports.Residents living in affordable housing most often have extreme socioeconomic disadvantages and a disproportionate burden of health disparities and health inequities.
In 2015, we re-structured our services delivery and launched Healthy Homes, a program focused more specifically on health and wellness. Healthy Homes is an innovative strategy to support interventions and services that promote housing access, retention and stabilization. We have five focus areas: The program has Youth Education, Food Insecurity, Health and Wellness, Adult Education, and Community Building. On-site Resident Services Coordinators provide regular scheduled programs like Active Kids/Teens, Rising Star, Get Motivated, Taste of Wellness, and Financial Fitness. We partner with Food for Lane County to deliver Summer Lunch, Extra Helping, and Senior Grocer programs on-site. Our resident volunteer programs help to provide volunteer opportunities for residents who want to gibe back in their own community.
Healthy Homes is also developing an innovative and unique approach to ensure residents have reduced barriers to health care by providing coordinated on-site programming and education utilizing Community Health Workers (CHW’s).Working with the local healthcare community we are developing programs that align health and housing in a more effective and coordinated way. Cornerstone is developing more effective referrals, increasing access to health opportunities, promoting healthy lifestyles, and delivering health related services in non-traditional health care settings like affordable housing.CHW’s work individually with households to directly support and impact a resident’s ability to access health care services,reduce health care costs andimprove health outcomes.
